I ______________________ accept my invitation to participate with Next Level Basketball during NEXT LEVEL ELITE TEAM season and agree to participate in practices as well as games from June-Aug . I understand that only a few can be selected and agree to full participation.
I understand that I have been invited to participate on the will play from Mar through May 31. I also understand that selection for the Summer Elite Travel team is not guaranteed solely because I participated in the Spring Season.
I also understand that I may also be invited to play during the summer evaluation period and that playing with the Summer Elite Travel Team is in addition to the spring seasons and may require an additional fee of $250 paid on May 15.

The NEXT LEVEL fee for the SPRING season is $500 HS and only $400 MS .

[ ] I Understand that if the fees may not be sufficient to meet all program needs and agree as a part of my being selected for the team, to participate in any and all fund raising. Failure to fund raise or failure to pay the rest of the fee may result in removal from the team. Once paid no fees can or will be refunded. Once a child has started practicing regardless of whether they play the full season, they are required to pay the entire fee and no partial refunds will be given.
